Referral System for Nutrition Consultation    
 
  • Purpose
    • To ensure proper administration of Nutrition care.
    • To provide standards when to conduct routine Nutrition Care Process.
  • Definitions
    • Nutrition Care Process -  a method for Clinical Dietitians applying standardized  process for providing care and  standardized  language in making decisions and provide high quality nutrition care.
    • Nutritional Assessment – to identify patient’s nutritional problems and high risk patients who need special attention as a basis for nutritional care plan. It includes collection and analysis of anthropometric, biochemical, clinical, dietary, and psychosocial data as well as consideration of the planned therapeutic management.
  • Policy statements
    • Clinical Dietitians shall conduct daily ward rounds of their assigned nursing units.
    • Clinical Dietitians shall provide diet counseling and monitor patient’s tolerance on the recommended diet.

 

  • Procedures
  1. All patients shall be screened by Nursing Staff using electronic system. (IPP: MAD:-01-03) Patients  identified with nutrition risk factors should be informed the Clinical Dietitian by paging. Official  Nutrition consultation / referral order should be done via electronic system through Order Entry -  Nutritional Referral.
  2. All In- patients for Nutrition Consultation Referral must be ordered via electronic by the Nursing Unit from 8:00 hours – 16:00 hours. Beyond these hours, such referral will be seen by the Clinical Dietitian on the next day (except week end).
  3. Every week end, On Call Clinical Dietitian is the one responsible for Nutrition Consult Referral by contacting via paging system. Urgent cases for consultation / Royal Family patients should be attended personally by Clinical Dietitian from 10:00 hours – 14:00 hours (Thursday – Friday)as ordered by physicians.
  4. All In- patients for nutrition referral on the day of discharge specifically beyond 8:00 – 16:00 hours should be provided with OPD appointment schedule and to be seen in Dietetics Clinic.
  5. All Out patients for nutrition referral, the Physician should fill up the Clinical Nutrition Referral Form.
