The Department of Medicine ​(DOM) at the King Khalid National Guard Hospital, King Abdulaziz Medical City-the MNGHA, Jeddah (Western Region), benchmarks with the organizational vision and mission statements encompassing quality, education, research, and leadership. DOM provides outpatient, inpatient, consultation services, and additional supporting services, including day case diagnostic and interventional procedures, training and education of medical students, interns, residents, and fellows from various specialties, and research and public awareness.​

Assalamu Alikum!

It is my great pleasure to welcome you to the Department of Medicine in King Abdulaziz Medical City-Jeddah.

We are made up of 12 divisions, and are committed to compassionate clinical care, educational excellence, and good clinical research. At the Department of Medicine our core mission is to undertake high-quality clinical care, outstanding education and research to provide the best possible health outcomes for our patients. We have 12 divisions that work collaboratively. We believe in fostering a culture of integration among all of our Consultant Physicians and staff, students and trainees, as well as our patients and their families.

Many of our department members are visionary leaders at institutional and national levels including the University and Saudi Commission for Health Specialty.

During the past challenging times through the global pandemic, Our Physician have seized the opportunity to contribute in the fight against COVID-19 with tireless effort to provide excellent patient care, adopted novel teaching approaches to educate our Students and Junior Physicians, to continue our academic mission.

Divisions:

Cardiology

The Cardiology section provides trainees (interns, residents, and fellows) with information on the latest available knowledge on cardiac diseases and their management, and provides them with maximum possible hands on the various investigative and diagnostics procedures available.

Moreover, the Cardiology section granted students from different universities elective Cardiology training. Structured activates are being held weekly covering a wide range of topics. The section is also fully involved in the ACLS, which is organized every two months.

In meeting the goals set out in the Mission Statement and Objectives of the Cardiology Section, the achievements are:

  • To provide efficient, cost-effective and highest possible standard of medical care, based on the current settings.
  • To provide CCU coverage 24 hours, 7 days a week.
  • To provide trainees (Interns, Residents, and Fellows) with information on the latest available knowledge on cardiac diseases and their management.
  • To provide trainees maximum possible hands-on training on the various Investigative and Diagnostic Procedures available.
  • To provide all necessary help to establish Cardiac catheterization /Surgery in coming months.
  • To contribute to the implementation of the Cardiac Center Program.
  • Continue running the ACLS for National Guard Staff and Health Care providers in the Western and Southern Region of the country. ​​

Dermatology

​The Dermatology section sees the full range of skin and venereological diseases. Outpatient clinics are held every weekday except Sunday at the hospital, and monthly at Jeddah, Madinah, and Taif Primary Health Care Centers.

The section manages both outpatients and inpatients. Outpatients are seen in eleven weekly clinics. Special procedure clinics handle internal referrals for treatments such as minor surgery, diathermy, chemocautery, cryosurgery, dermabrasion, laser hair removal, and intralesional injection therapy. Urgent cases are accommodated early in the morning, while acute venereological cases are seen immediately upon request.

Inpatients typically present with serious conditions that cannot be managed outpatient, or are referred by other departments for shared care.

Teaching junior staff, medical residents, family medicine residents, and general practitioners is a key responsibility. The section is accredited by the Saudi Council for Health Specialties for Saudi Board training in Dermatology.

General Internal Medicine

GIM section is the core of Department of Medicne which strives to improve qualit​y of patient care delivered at KAMC.

  • Implementing JCI (Joint Commission international) standards into every day GIM clinical practice.

  • Improving the safety and quality of patient care, with emphasis on medication safety and infection control.

  • Implementing VTE Prophylaxis

  • Improving patient care outcomes, with ultimate reduction in morbidity and mortality.

  • Improving clinical documentation

  • Implementing proactive discharge planning, and reduce inpatient length-of-stay with avoidance of re-hospitalization

  • Identifying priorities, strengths and weaknesses of GIM services.

Endocrinology

The Division of Endocrinology at King Khalid National Guard Hospital is committed to providing the highest level of care for patients with disorders of the endocrine system. The goal of the section head of Endocrinology is to unify the division and integrate the following three-fold mission:

  • To provide excellence in patient care
  • To offer outstanding educational opportunities for medical students, residents, and fellows
  • To perform cutting-edge basic and clinical research on endocrine diseases, diabetes, thyroid cancer, and metabolic bone disease

The Endocrinology service holds multidisciplinary clinics including:

  • Thyroid Cancer Clinic – A tertiary referral clinic for the Western Region, offering weekly consultations with specialists from Endocrinology, Head & Neck Surgery, and Radiation Oncology
  • Gestational Diabetes Clinic – A combined clinic with Endocrinology, Obstetrics, and a diabetic educator to assess both maternal and fetal health
  • Adolescent Clinic – A monthly clinic where adult and pediatric endocrinologists collaborate to transition adolescent patients to adult care​

Gastroenterology

The section of Gastroenterology is committed to provide the highest quality of clinical care available in the Region in the outpatient and inpatient ​settings, outstanding teaching to residents and peers, and ongoing research efforts in clinical and education methods are in progress.

Gastroenterology section provides services for all eligible patients referred to King Khalid National Guard Hospital, Jeddah for secondary and tertiary gastro-intestinal and/or hepatobiliary medical problems to cover;

  • Gastrointestinal diseases

  • On-call gastro-hepatology coverage service

Nephrology

Mission Statement and Objectives

The Renal section provides dialysis services, inpatient care, and outpatient clinics. It is currently operated by three full-time Consultant Nephrologists, with additional part-time support from the Departmental Chair for academic activities and backup coverage.

The Nephrology section is responsible for delivering patient care, as well as training residents in the Saudi Board, interns, and medical students. A core mission is to initiate clinical research and involve renal physicians and trainees as part of their education.

All Nephrology activities are aligned with King Khalid National Guard Hospital regulations and the Department of Internal Medicine, supporting optimal patient care and medical training for young Saudi physicians.

The section serves patients from the dialysis unit, emergency room, and outpatient clinics. Clinical Nephrology provides consultations for inpatients with varying degrees of renal impairment and promotes referrals for outpatient follow-up. Preventive strategies include early planning for AV-fistula creation and initiating erythropoietin therapy—both proven to reduce national healthcare costs for ESRD and Pre-ESRD care.

Outpatient follow-ups for patients undergoing CAPD and CCPD are conducted monthly at the Peritoneal Dialysis Clinic in Clinic One, held every Wednesday afternoon.​​

Neurology

​The Neurology section continues to comply with the mission of NGHA that is to provide the highest quality of neurology services to the National Guard personnel and their eligible dependants as well as other eligible patients. Promote research, training and neurological education in NGHA and in the Kingdom as a whole.

There have been an increased number of inpatients and outpatients seen in Neurology section, as well as in EMG, NCS and EEG. Dr. Attar continues to establish movement disorder clinic.

Protocols for most common neurological disease and patient education materials were established.

The section is in need of Stroke Unit, as it is mandatory due to the large number of stroke patients, but still clinical pathway is not followed due to lack of manpower

Psychiatry

Mission

The mission of this service is to promote mental health within hospital and community settings, increase awareness, and encourage care for optimal psychological functioning. It provides comprehensive assessment and management for patients across all ages and departments, including military psychological support.

The section offers high-quality clinical care in both inpatient and outpatient settings, excellent educational opportunities for residents and peers, and conducts ongoing research in clinical and educational methods.

It actively supports outreach clinics scheduled within the community.

In-patient Care

Patients are admitted to the general medical bed pool without a dedicated psychiatric ward. The average inpatient load is 5 patients. Care is provided within a secure therapeutic environment using various therapeutic techniques to support recovery.

Liaison Service

Services are extended to Oncology, Pediatrics, General Medicine, Geriatrics, Obstetrics, and Rehabilitation. This includes assessments, treatment, follow-up, psychometric evaluations, and cognitive assessments.

Special Procedures

  • Electroconvulsive therapy: 3 times per week
  • Psychometric testing: once per week
  • Cognitive behavior therapy: once per week
  • Psychotherapy: once per week​

Respirology

​It is an aim to provide a comprehensive and cost-effective outpatient and inpatient service for the critically ill and those with full spectrum of pulmonary disorders including:

  • Asthma and chronic airway diseases

  • Pulmonary Tuberculosis and other chest infections

  • Peri-operative evaluations for patients with respiratory disorders

  • Interstitial lung diseases

  • Acute and chronic respiratory failure

  • Lung Cancer

  • Obstructive Sleep Apnea and other sleep disorders including Insomnia

  • Pulmonary Hypertension.

With the strength in the number of consultants improving, the section will be formerly applying for joining the Saudi Council Pulmonary Fellowship Program for the Western Region this year. Similarly once the Equipment is received, the returning Fellow can start the facility of Interventional Bronchoscopy.

Rheumatology

Offers highly specialized care and plan for patients with MSK problems.

Rheumatology has excelled in the area of research, education and patient care, as they continue to provide comprehensive evaluation and management of patients with spectrum of inflammatory disease. The section is committed to provide outstanding medical care for rheumatological diseases in the most compassionate way.

The service has advanced therapeutic modalities for MSK diseases like RA, SLE, spondyloarthopadies. These medications are DMARDS, Biologic like Infliximab and Etanerecept, Rutuximab, etc.

Day case facilities are available for patient scheduled for biological drug infusion.

The Rheumatology section provides seven outpatient clinics, and emergency consultation for 24 hours all over the week. All new patients with a systemic rheumatic disease (e.g., RA, SLE, AS) are treated and managed appropriate in the OPD. All residents rotating in the section have the chance to see and manage all these patients.

Fellowship programme of Saudi Board has been accredited last year and one fellow has joined the program.

The section is active in conducting research and currently involved in International Multicenter clinical trial of Toclizumab therapy in RA patients.

The Rheumatology section had a strong and a very productive year.

  • A new initiative to improve the service was undertaken. The section staff members provide educational experience for rotating residents and fellows.

  • The section continued a robust inpatient and outpatient practice that resulted in a year of rapid growth and development in all areas.

  • Our areas of clinical expertise include the treatment of Systemic Lupus Erythematosus, Sjögren’s syndrome, rheumatoid arthritis, inflammatory muscle diseases and common arthritic problems, such as osteoarthritis, Fibromyalgia and osteoporosis.

  • We deliver personalized care to each of our patients who benefit from the most recent diagnostic and therapeutic care of rheumatic diseases.

  • The Section of Rheumatology contributes in the teaching of medical students in COM-J, as PBL curriculum: two of the rheumatology section consultants have now joined COM-J as staff members in the university.

  • The Section of Rheumatology is committed to provide outstanding medical care for rheumatological diseases in the most compassionate way in an environment enriched by teaching and research with good cooperation between all staffs in the section of rheumatology.​

Infectious Disease

Mission Statement and​ Objectives

  • To optimize the management of patients with suspected/proven infections at King Abdulaziz Medical City – National Guard Health Affairs, Jeddah.

  • To provide teaching of infectious disease to trainees within the department.

  • To carry out limited, clinically oriented research in the area of infectious disease.

The section of Infectious Diseases is providing a hospital wide consultation service for outpatients and inpatients. It also performs an inpatient services for HIV infection. These services are conducted through a rotating trainee, usually a resident, with a contribution of rotating fellow.

Consultation service to all departments is provided during regular hours, after hours and on weekends. Residents and fellows will have the chance to learn the management and follow-up with patients with HIV, tuberculosis, brucellosis, osteomyelitis and harware related infections.

Interns, residents, fellows and consultants are contributing to the various presentations. In the first Sunday of every month, house staff will have a microbiology round in the microbiology laboratory. They will be familiar with the different gram stain of gram positive and gram bacteria as well as cultures and antibacterial susceptibility. They will also be familiar with the different fungi.

The section is accredited by the Saudi Council of Health Specialties as a training center for the Fellowship program in Infectious Diseases for July 2005.

Upon completion of the Infectious Diseases rotation, trainees are expected to be familiar with the following aspects of infectious diseases:

  • A practical approach to the febrile patient including the immunocompetent & immunocompromised host

  • Major infectious syndromes (community acquired & hospital related), their etiology, pathogenesis, clinical presentation, diagnosis and management

  • Common anti-infective agents, their cost, spectrum of activity, toxicities, interactions, special adjustments and precaution.

  • The role of the microbiology lab in the management of patients with infectious diseases.

Interns, residents and fellows are requested to download the ID resident file located in the desktop of the main computer in the Department of Medicine. It contains an updated and comprehensive review for the most common infectious diseases syndromes (community acquired pneumonia, nosocomial pneumonia, UTI, meningitis, abdominal sepsis, ID emergencies, Fever in ICU, infections in solid organ transplantation, febrile neutropenia guidelines, prosthetic joint infections, septic arthritis, osteomyelitis, invasive candida infections, aspergillosis, TB & HIV).

Consultants will guide trainees for specific references related to certain infectious diseases topics as they arise during the rotation. Harrison textbook of Internal Medicine is always a comprehensive reference for any Infectious Diseases question.

Department Capacity:

Department of Internal Medicine has more than 264 employees under its administration, including 63 Consultants. Scholarship positions in different Internal Medicine subspecialties have been cleverly planned and approved to strengthen expected expanding services in the future.

In 2020, the total patient admissions for all subspecialties in the Department of Internal Medicine had reached 4,130. The total of outpatient visits to all subspecialties reached 61,805. The medical subspecialties have received a total of 5,250 referrals and 4,125 consultations.
